Information clause for forum participants

Pursuant to Article 13 of Regulation (EU) No. 2016/679 of the European Parliament and of the Council of 27 April 2016 on the protection of individuals with regard to the processing of personal data and on the free movement of such data and repealing Directive 95/46/EC – General Regulation on data protection (Official Journal of the European Union L 119/1 of 04.05.2016) I hereby inform you that:

  1. The Controller of your personal data is Adam Mickiewicz University in Poznań with its registered office at 1, Henryka Wieniawskiego Street, 61-712 Poznań.
  2. The controller of personal data has appointed a Data Protection Officer, who can be contacted via e-mail address: iod@amu.edu.pl.
  3. Your personal data will be processed in order to register, organize, conduct and publish the materials from the Forum on The Future of Morality. 
  4. The legal basis for the processing of your personal data is Article 6(1)(a) of the General Data Protection Regulation of 27 April 2016 (hereinafter referred to as RODO)
  5. Provision of the personal data is voluntary, but in the  scope required by the Organizer in order to register participants and enable them to participate in the conference it is obligatory. Failure to provide this data excludes participation in the conference.
  6. Your personal data will be stored for a period necessary to the above-mentioned purposes.
  7. You have the right to:
  1. access your data, including the right to obtain a copy of the data – on the legal basis of art. 15 RODO,
  2. request rectification (correction) of your personal data – on the legal basis of art. 16 RODO ,
  3. delete the data – within the premises and under the conditions specified in art. 17 RODO,
  4. restrict the processing – within the premises and under the conditions specified in art. 18 RODO,
  5. transfer personal data – within the premises and under the conditions specified in art. 20 RODO,
  6. object to the data processing – within the premises and under the conditions specified in art. 21 RODO,
  7. lodge a complaint to the supervisory authority (the President of the Office for Personal Data Protection),
  8. withdraw consent for data processing. You have the right to withdraw your consent for personal data processing at any time. Withdrawal of consent does not affect the lawfulness of the processing which was carried out on the basis of consent before its withdrawal. The consent may be withdrawn in the same form in which the consent was given. Please send information on the withdrawal of consent to the following address:

  1. With regard to your personal data, decisions will not be taken automatically and will not be profiled, in accordance with Article 22 RODO.
 

This consent may be withdrawn at any time by sending a message to fom@amu.edu.pl. Withdrawal of consent does not affect the lawfulness of the processing of personal data, which was carried out on the basis of consent before its withdrawal.

I am aware that providing data is voluntary, but necessary to the purpose for which they were collected.
